Hepatitis A #2 can be given at this well visit or at 2 years

MMR and Varicella may be given in a combination vaccine called MMR-V

The following vaccines will be recommended and discussed:

Tdap (Tetanus, Diphtheria, Pertussis) (10-12 yrs.) Required prior to 6th grade entry in KY

Meningococcal vaccine (single dose) (11yrs and older). Required prior to sixth grade entry in KY. Second dose at age 16 years (new in 2011).

Varicella #2(Chickenpox) (if not given prior). The second dose is required before sixth grade entry in KY.

HPV (Gardasil) 3 dose series

All children over age 6 months Discuss annual influenzae vaccine with your provider
