Dr. Cindy Spicker is Retiring

After 35 years of holding tiny babies, reassuring new parents, and watching generations of children grow, I am stepping away from my pediatric career with a full heart. After much reflection, I will be retiring on July 1, 2026. Being a pediatrician is a calling like no other. To play even a small role in shaping our future has been the honor of a lifetime.
I have been especially fortunate to spend most of my career at Pediatric Associates of NKy. Throughout my 31 years here, I have had the privilege of working alongside many intelligent, compassionate and deeply dedicated colleagues. Our office truly feels like a family. We celebrate milestones together, support one another through challenges, and share a common purpose: providing the best possible care for our patients and their families
One of the greatest joys of pediatrics is the continuity—caring for patients as they grow from newborns into confident young adults. There is something profoundly special about watching a child you once held in your arms return years later as a parent, trusting you to care for their own newborn. Moments like these are gentle reminders of the deep relationships and lasting connections that make this profession so meaningful. In addition to providing direct patient care, it has been a privilege to help train future medical providers by mentoring young adults early in their journeys. Seeing them grow into compassionate professionals has been incredibly rewarding.
I am deeply thankful for my husband and our three children, who supported me unconditionally throughout the years, even when the demands of my work meant time away from home. Their love, patience, and encouragement made this journey possible. As I close this chapter, I do so with immense gratitude—for the families who entrusted me with their children’s care, for the colleagues who became lifelong friends and for a career that has given me more joy and purpose than I ever could have imagined. I will carry your stories and smiles with me always.

Fondly,
MYTH OF THE MONTH: "RAW MILK IS BETTER THAN PASTEURIZED MILK"

Raw (unpasteurized) milk is sometimes marketed as a more “natural” or more nutritious alternative to pasteurized milk. But the idea that raw milk is healthier doesn’t hold up well when you look at safety and nutrition evidence.

What pasteurization means: Pasteurization is a process in which milk is heated to a specific temperature for a specific amount of time and then rapidly cooled in order to kill harmful bacteria and reduce the risk of foodborne illness.

Raw (unpasteurized) milk can carry dangerous bacteria. Because it has not been treated to kill harmful microbes, raw milk can lead to infections from Campylobacter, Brucella, Listeria monocytogenes, Salmonella, and E. coli. These infections can be serious even in otherwise healthy people, and raw milk exposure can be particularly harmful to children. Certain types of E. coli can cause life‑threatening kidney disease in children. Even healthy older children can develop severe diarrhea and dehydration from these bacterial infections. Some of these bacteria are especially dangerous during pregnancy. For example, infections such as Listeria monocytogenes can be passed from a pregnant person to the unborn infant and may result in birth defects, miscarriage, stillbirth, or serious infection in the newborn.

Key nutrients are mostly unchanged:
A common claim is that pasteurization “destroys” the nutritional value of milk. However, multiple studies have shown that pasteurized milk contains essentially equivalent nutrients compared with unpasteurized (raw) milk products. In other words, pasteurization makes milk safer without meaningfully reducing its core nutritional benefits. The nutrients children commonly rely on milk for remain largely intact after pasteurization:
-Minerals such as calcium and phosphorus are unchanged, and these are among the most important nutrients milk provides for growing children.

-There may be some deactivation (denaturation) of proteins, but the overall difference is small and does not meaningfully reduce milk’s protein value in the diet.
-A few vitamins—especially vitamin C and some B vitamins, may decrease somewhat. However, milk is not a primary source of vitamin C or many B vitamins in most diets, so these small reductions generally have little nutritional impact.

Risk vs. Benefits:
In summary, the potential risk of infection from bacteria in raw or unpasteurized milk outweighs any claimed benefits, and we recommend against the consumption of raw milk.

The holidays can be exciting but also stressful for children. Keeping regular routines for sleep, meals, and physical activity helps children manage holiday stress. Each child is different and may handle crowds/new people/excitement differently. Prepare your child(ren) for events by sharing basics (who, what, why, how long, and expectations) ahead of time. Encourage your children to discuss how they feel about events. This knowledge and open communication helps anyone navigate new or different experiences! If they have big emotions, help them manage by being supportive, listening, help them to solve problems and teach them simple stress management techniques like deep breathing.

If you are noticing persistent behavior challenges or changes, let us know. Symptoms of anxiety and depression can include excessive worrying, avoiding activities they used to enjoy, frequent stomachaches or headaches, trouble sleeping, irritability, or seeming sad most of the time. Younger children may show more tantrums or clinginess, while older children and teens might withdraw from family or friends. Early help can make a big difference, and we can provide evaluations and guidance at Pediatric Associates.

Maintaining Routines Even During Summer Break

With summer break approaching and the school year coming to an end, it is important to start thinking about your family's routine for the summer. Having a set schedule and somewhat consistent routine is important for children’s mental health. Routines help foster a sense of safety, stability, and predictability, which are key for emotional well-being as well as help kids develop a sense of responsibility. Although kids are excited to be out of school, they will continue to benefit from a regular routine.

The Benefits of Routine:

Creates a sense of security: having an idea of what to expect each days helps kids feel safe and in control

Reduces stress and anxiety: predictable routines can decrease worry and stress

Build confidence and independence: when kids know what to do next, they feel more capable and responsible

Supports better sleep: regular and consistent bedtime routines helps kids fall faster

Improves behavior: routine reduces power struggles, emotional meltdowns, tantrums and impulsiveness

Helps manage transitions: moving from one activity to the next is easier with a predictable routine

Helps support mental health treatment: routines can make taking medications, counseling, and self care more consistent for children with ADHD, anxiety and depression

Although sticking to a routine will be difficult on some days, unexpected things will pop up, and you might just have an “off day”, it is important to not let these minor setbacks throw everyone off. Routines look different for each family and change day to day, but having a set of daily expectations will help foster your child’s mental health and aid in the transition back to school come fall.

The cold weather is officially upon us but don’t let that stop you from getting your kids and family outside! Although it takes more prep to get outside in the cold, the health benefits are worth it. Make getting outside a habit all year long!

The colder months often bring feelings of isolation, lethargy, or even Seasonal Affective Disorder (SAD). However, stepping outside—even in the cold—can do wonders for your mental well-being.

Here’s why braving the chill is worth it and how you can make it a habit this winter:

Boosts Mood- Exposure to natural light, even on cloudy days, helps regulate your body’s production of serotonin and dopamine; these are your “feel good” neurotransmitters linked to positive moods.

Promotes Sleep- Being outside can lead to regulating your levels of melatonin to help with sleep initiation at night.

Reduces Stress/ Improved Patience- Spending time in nature reduces cortisol levels, alleviating stress and promotes calmness/relaxation.

Improves Focus and Creativity- Exercise outside helps with increasing oxygen flow to the body as well as increasing endorphins which, in turn, enhances cognitive function and improves focus. A quick 15 minute walk can help clear the mind and promote mental health/clarity. Move your body to get the rush of feel good neurotransmitters!

Physical Health- Playing outside in the winter can actually boost your immune system! Exercising outside this winter can also reduce your risk of chronic health conditions.

Although it is cold, stepping outside is one of the simplest ways to care for your mental health. Whether it’s a short walk, a snowy adventure, or just a few deep breaths of fresh air, the benefits of being outdoors—even in the cold—are undeniable. Even if you get bundled up and only spend 5 minutes outside! So, bundle up, embrace the chill, and make the most of the season. Your mind and body will thank you.

As the winter season sets in, it's essential to ensure our little ones stay active and healthy despite the chilly weather. At PANK, we understand the importance of maintaining both physical and mental well-being during this time. Here are some fun activities that can help your children stay healthy and happy throughout the winter months:

           1.

          Winter Wonderland Walks:

Encourage outdoor walks in your local winter wonderland. Bundle up in warm layers and explore the beauty of snow-covered landscapes. Walking is an excellent way to keep both the body and mind active while enjoying the crisp winter air.

           2.

          Snowy Adventures:

Embrace the snow by engaging in classic winter activities like building snowmen, having snowball fights, or creating snow angels. These activities not only provide a great workout but also foster creativity and teamwork.

When the weather is too cold to venture outside, bring the fun indoors with dance parties. Play your child's favorite music, clear a space, and let them dance away. Dancing is a fantastic way to enhance coordination and cardiovascular health.

Introduce your children to winter sports like ice skating or sledding. These activities not only promote physical fitness but also offer a chance for family bonding. Remember to prioritize safety by using appropriate gear and choosing age-appropriate slopes.

Healthy Cooking Together:

Use the winter season as an opportunity to explore healthy recipes with your children. Cooking together can be a fun and educational activity. Choose nutritious ingredients and involve them in the preparation process to instill healthy eating habits.

Reading Adventures:

Curling up with a good book is a cozy and intellectually stimulating activity. Create a reading nook with blankets and pillows, and let your child explore the world of imagination through books. Reading together also strengthens the parent-child bond.

           7.

          Nature Scavenger Hunt:

Even in winter, there are fascinating things to discover in nature. Organize a scavenger hunt by creating a list of winter-themed items for your child to find. This activity promotes outdoor exploration and observation skills.

Remember, keeping our children active and engaged during the winter months contributes to their overall well-being. If you have any specific concerns or questions about winter activities for your child, don't hesitate to reach out. Wishing you and your family a healthy and joyful winter season!

Quick Guide to Meditation

When feeling anxious, a quick meditation can be helpful! Here are some ways to start:

Sitting meditation- Practicing this for just 3-5 minutes can be very beneficial. To start, sit or lie in a comfortable position. Close your eyes and try to focu on your breathing. Try not to focus on your thoughts, but rather allow them to simply be present. Think of your thoughts as clouds passing by in the sky, or leaves floating down a stream.

Breathing techniques- 4-7-8 breathing is an easy way to take some deep, calming breaths. Place one hand on your stomach and inhale through your nose for 4 seconds, feeling your belly expand with the inhale. Hold the breath for 7 seconds. Release the breath through your mouth for 8 seconds feeling your belly contract with the exhale. Repeat as needed until you are feeling more calm and relaxed.

Body scan or progressive muscle relaxation- Find a comfortable place to sit or lie. Focus on your toes; wiggle them, and then let them tense for a few seconds and then relax. Use the same process on all parts of your body, starting from your toes and moving upward to your calves, thighs, abdomen, chest, shoulders, arms, neck, etc. Move through each muscle group tensing for a few seconds and then releasing. Notice the tension leaving your muscles as you release.

If you don't feel ready or able to do these exercises on your own, there are plenty of guided meditation videos on YouTube to help!

PANK Blog: Resiliency

Resiliency can be defined as the ability to bounce back after a difficult encounter or situation. Resiliency can also be viewed as "toughness." We want our children to be resilient, especially when it comes to the many hardships that adolescents face nowadays.

Below are some characteristics that resilient people possess:

-Resilient people are flexible. In order to be resilient, we must be willing to adapt to our surroundings and understand that life sometimes does not go as planned, but that is okay.
-Resilient people are teachable. Being willing to learn new skills to cope with changing or unexpected situations is another key factor in building resilience.
-Resilient people are resourceful and seek solutions. Instead of complaining that things are hard or that we don't have what we need to get things done, resiliency is the ability to use what we do have to change our situation and to find solutions to our problems.
-Resilient people set realistic expectations. This is especially important. Unrealistic expectations can lead to major disappointment and can discourage us from continuing toward our goals. Setting realistic and attainable expectations can help us to be more prepared for what is to come.

Ways to improve overall resilience:

-Push past your comfort zone. Do something that worries you, and work on accepting anxious feelings and allowing yourself to be okay with those feelings.
-Set small scale realistic goals that are helping to move you toward bigger goals. This is helpful because we feel accomplished when we meet small goals, and that encourages us to continue toward the bigger goals.
-Change your mindset. Think of things that are scary or worrisome as challenging. Change the language you use with yourself to describe your thoughts and feelings to be more encouraging and productive.
-Utilize coping skills such as drawing, cooking, listening to music, journaling, spending time with your support system.
-Practice self-care. Do something that you enjoy, take care of your physical and personal health and hygiene, practice good sleeping and eating habits.

The FDA and CDC have authorized the use of the Pfizer AND Moderna vaccine for young children and babies down to age 6 months. We have the vaccine on order for our patients and will be able to make appointments for it as soon as we receive our shipment. There are many people who will say that kids do not get sick from COVID. While kids do tolerate the infection much better than adults, most people do not realize that the past two years of COVID have been harder on kids than the worst flu years in terms of hospitalizations and mortality. And kids are very proficient at spreading COVID. Both vaccines have shown efficacy and excellent safety records. For these reasons, we will be vaccinating the youngest members of our own families. Feel free to contact us if you have questions and look for an announcement soon on when you can schedule an appointment to have this potentially life-saving shot.

Many pharmacies are now dealing with staffing shortages. This has resulted in longer waits for prescriptions and shorter hours, especially on weekends. If you need a prescription in the evenings or on weekends please verify your pharmacy hours. Also we have learned that Kroger Pharmacy no longer takes GoodRx coupons but have started their own medication savings plan.

Monkeypox: Something Else to Worry About?

For a pandemic weary world, the prospect of another strange virus spreading around the world can be unsettling. Monkeypox, an endemic illness in West and Central Africa, has been recognized in several countries around the world and many of the cases do not seem to be related. What this means is that it has probably been here longer than we initially thought. Fortunately, the strain being seen outside Africa is the less dangerous version. We are not saying to run out and have a monkeypox party with an infected friend, but this is not an infection to panic over. CDC is watching the outbreak closely and, while it is widespread, it is not causing many bad infections and risk is felt to be low to the general US population. You can learn more about monkeypox

PANK Blog: Summer Safety - Tick Bites

It's that time of year again. The weather begs us to get outside and get active. This is wonderful for our patients, but this outdoor time also brings a few more injuries, accidents, and exposures to nature. In the coming months, we will share info on injury prevention to help our patients stay safe and healthy.

In this blog post, we will discuss how to remove ticks and what signs to watch for that might indicate Lyme disease.

          How to Remove Ticks:

The most important thing to keep in mind when removing ticks is to do it as soon as possible. The longer a tick stays attached to the skin, the higher the risk of disease transmission. Most often, if a tick is removed within 48 hours of attachment the risk for disease transmission is very small. Here are the steps to follow when removing a tick:

Use tweezers: Using a pair of fine-tipped tweezers, grasp the tick as close to the skins surface as possible.

Pull upward: Gently and steadily, pull the tick upward without twisting or jerking it. This will help to avoid leaving the tick's mouthpart in the skin.

Clean the area: After removing the tick, clean the area with soap and water or an antiseptic solution.

Save the tick in a plastic bag (can be helpful if you develop symptoms later)

Symptoms to watch for after removal of tick:

Rash, specifically a bullseye like rash

Flu-like symptoms (fever, chills, headache, body aches, fatigue, joint pain)

           Joint pain or swelling

Follow up with us in the office if any of the above symptoms develop.

Continuity of care is a central component of the patient-centered medical home. There are many benefits to going to one place for all your child’s needs, whether for illness, behavioral health, sports physicals, or wellness checks. Here at Pediatric Associates, we pride ourselves in offering same day availability for illness 5 days a week at all three locations and on Saturdays at our Crestview Hills location.

Within our practice, you have many choices of providers, times and locations. There are situations where you will benefit by seeing the same provider for a given condition – such as for recurrent infections, weight or developmental checks, behavioral health issues, or chronic illness such as migraines, asthma, or abdominal pain.

When you schedule an appointment, please feel free to ask for a specific provider, especially if you have been working with them on a particular issue. This will enhance your experience and reduce redundant conversations. Our staff may encourage this when you call. Thank you for allowing us the privilege to help care for your children!

It's that time of year again--the weather begs us to get outside and get active. This is wonderful for our patients, but this outdoor time also brings a few more injuries, accidents, and exposures to nature. In the coming months, we will share info on injury prevention to help our patients stay safe and healthy. This month, let's talk about bike helmets! 75% of bike fatalities would be prevented with a helmet. Everyone should wear a helmet every time they are on a bike, even if it's a quick jaunt across the street. In the "Community Resources" section of our website, we have a

PANK Blog: Decrease Screen Time

Establishing a Goal to Decrease Screen Time!

The American Academy of Pediatrics (AAP) calls for no screen time for children until 18 to 24 months and says kids ages 2 to 5 should get an hour or less of screen time per day.

When a child spends multiple hours a day on things that are overstimulating such as ipads, TV, or cell phones, it makes it hard for them to maintain focus on things that are not as stimulating such as everyday conversations, lectures at school, or nature. When a child watches a screen, their brains will grow to crave that level of entertainment in order for their brain to maintain attention. However, the real world will never be as stimulating as a screen which leads to inattention in daily life. Babies and children will learn much more about their world by watching you or other people rather than watching a screen. When your kids watch you talk with a spouse, cook, or even clean the kitchen, they are learning important social and functional skills. When kids watch a screen out in public, this is impairing their ability to learn how the world around them works and how they can fit into social constructs of the environment.

           Ways to decrease screen time!

Getting children outside promotes the innate exploring and brain development that the brain needs as well as serving as physical exercise. It also teaches independent play to let kids explore nature; no matter the age. Letting kids get messy outdoors helps with sensory integration as well as builds confidence!

Cooking is another great way to teach kids independence as well as teach them great life skills. As well as bonding with you!

Another great way to keep kids active outside of screens is keeping them involved in a sport or exploring other great venues around our city such as the zoo, aquarium, or a park.

Breastfeeding FAQs

           Latch/pain

Breastfeeding may cause nipple irritation as your body adjusts. Application of lanolin cream after every feed can help to prevent cracking and bleeding.

Your nipple shape should look the same before and after your baby feeds. If your nipple is flat or misshapen after your baby unlatches, you may want to see a lactation counselor for help adjusting the latch.

           Feeding frequency/expectations

Breastfed infants will show signs of hunger cues by tightening their fists, bringing fists to mouth, opening and closing mouth, rooting.

Cluster feeding is when infants want to feed every hour. This often happens in the evening hours and it is their way of increasing your supply to meet their needs.

           Pumping and return to work

Practicing with your pump before returning to work is recommended. You may pump a different volume of milk at different times of the day – that is normal! Hydration and good nutrition will help to maintain your supply.

Supply – do I have enough? How do I know?

If you baby is gaining weight well, having seedy yellow stools and urinating at least 6 wet diapers/day, then you can feel confident that you are supplying enough breastmilk for your baby. After feeding, infants will look more relaxed and satisfied.

           Timeframe when milk comes in

Initially after delivery, you will be able to hand express some drops of colostrum. Colostrum is the first milk produced that is rich in protein and antibodies!

On average, a mother’s milk will come in on day 3-5 post-delivery. If you have a c-section, it may take a little longer for milk to come in completely.

           How to increase supply

           Skin to skin

           Hydration and good nutrition

Frequent emptying of the breast – either with direct feeding or pumping

           Signs of mastitis, candida, oversupply

If you develop fever, breast pain or redness of the skin on your breasts, then call your doctor.

           Ok to use pacifier

Pacifiers can be used and still preserve a healthy breastfeeding relationship. They are also recommended for safe sleep!

           Support system/ask for help

Breastfeeding can be challenging! It is always helpful to reach out to a trusted family member, lactation consultant or pediatrician if you have any questions. We are always here to help!

We asked a few of our providers for advice on keeping kids moving even when the weather isn’t favorable. Here are their answers:

          From Dr. Spicker:

“Stay active and have fun inside with Cosmic Kids Yoga!
Bundle up and take a walk in your neighborhood!”

          From Dr. Hsu:

“Too cold to play outside?...turn on your favorite tunes and have a dance-off!!!”

          From Dr. Voet:

“Dress warm and see a different perspective on your favorite trail in winter! Also,
“The Cincinnati Zoo is less crowded in winter and has plenty of indoor exhibits to warm up in

          Dear Patients, Families and Friends,

Since the announcement of our plans to retire a year ago, and especially in the past few weeks and days, we have been the recipients of so many thoughtful cards, notes and social media posts sent by you, offering best wishes and reminiscing about your experience with us at Pediatric Associates. It has been, quite simply, overwhelming! It would be impossible to respond to each note individually, but we do want to acknowledge that we have seen these notes and had so many emotional responses to them.

We are thrilled that the experiences you have had with us, and our team at Pediatric Associates, have been as meaningful to you as they have been for us. Moving forward, we are excited about the talented colleagues that have replaced us and the direction of the practice in this challenging healthcare landscape. We know you are in good hands for many, many years to come.

In closing, we thank you from the bottom of our hearts for your kindness and fond memories! You have provided us with reflections to last a lifetime and we wish you endless years of happiness and good health!

With Deepest Gratitude,

We are aware of the extended wait times in the Cincinnati Children's emergency department. There are a lot of patients with mental health concerns who are needing immediate assistance in the ER. We want to take the time to offer outpatient resources to ensure you feel your mental health needs are being met. Please call the office if you feel your concern needs to be triaged and we can assist you in making the decision about what is the best place for your child to receive help. Although the ER is always warranted for emergency crisis management, we want to avoid this route if possible.

Please reach out to our behavioral health team if you have questions or concerns about your child/teen. We are available to talk with you about many mental health needs including ADHD, anxiety, suicidal thoughts, self harm, or depression. We offer therapy as well as psychiatric medication management. Our dedicated and experienced team are ready to help support you and your child through these uncertain times.

Mental Health Month is celebrated each year in the month of October. This month encourages all of us to think about our mental health and wellbeing, regardless of whether we may have a lived experience of mental illness or not. It also gives us the opportunity to understand the importance of good mental health in our everyday lives. A growing trend in 2022 is the impact social media may have on our youth and their mental health. According to Stephen Balkam, CEO of the Family Online Safety Institute, In high-tech families, the average age of kids that get cell phones is 7 ½ years old.

Tongue Ties and Lip Ties – When Should You Worry and Who Should You Consult?

Tongue-tie (ankyloglossia) is a condition present at birth that restricts the tongue's range of motion.
With tongue-tie, an unusually short, thick or tight band of tissue (lingual frenulum) tethers the bottom of the tongue's tip to the floor of the mouth, so it may interfere with breast-feeding. Someone who has tongue-tie might have difficulty sticking out his or her tongue. Tongue-tie can also affect the way a child eats, speaks and swallows
Sometimes tongue-tie may not cause problems. Some cases may require a simple surgical procedure for correction. The gold standard procedure (frenotomy or frenulectomy) uses a spatula device to lift the tongue and simple scissors to cut the thin webbing underneath. This can be done by pediatricians in the birth hospital or office, and causes minimal pain or bleeding. Some dentists offer laser surgery at very high prices to remove the webbing. We have seen babies in severe pain after this procedure sometimes resulting in oral aversion (refusing to eat).
Signs and symptoms of tongue-tie include:

Difficulty lifting the tongue to the upper teeth or moving the tongue from side to side

Trouble sticking out the tongue past the lower front teeth

A tongue that appears notched or heart shaped when stuck out

           Breast-feeding problems.

Breast-feeding requires a baby to keep his or her tongue over the lower gum while sucking. If unable to move the tongue or keep it in the right position, the baby might chew instead of suck on the nipple. This can cause significant nipple pain and interfere with a baby's ability to get breast milk. Ultimately, poor breast-feeding can lead to inadequate nutrition and failure to thrive. Frenotomy or frenulectomy alone are not always sufficient to resolve the latch issues and mothers should continue to work with lactation consultants to maximize comfort and milk transfer.

           Speech difficulties.

Tongue-tie can interfere with the ability to make certain sounds — such as "t," "d," "z," "s," "th," "r" and "l."

           Poor oral hygiene.

For an older child or adult, tongue-tie can make it difficult to sweep food debris from the teeth. This can contribute to tooth decay and inflammation of the gums (gingivitis). Tongue-tie can also lead to the formation of a gap or space between the two bottom front teeth.

           Challenges with other oral activities.

Tongue-tie can interfere with activities such as licking an ice cream cone, licking the lips, kissing or playing a wind instrument.

A word about lip frenulums and lip tie. Many babies are born with a prominent upper frenulum that appears to cause a gap in their teeth once the teeth come in. There is rarely a reason to cut or laser this normal and common tissue. Studies are very mixed as to the benefits of surgery on this tissue. There are studies that demonstrate cutting heals better than laser surgery. If you think your baby has a restrictive upper frenlum, please give us a call. We will exam your child and then we may refer you to a breastfeeding specialist or ear, nose, and throat doctor. But more often than not we recommend observation.

September is suicide prevention month. Did you know that 15 percent of teens and young adults thought about suicide last year? Chances are, you know a child, teen, or

young adult that is having these thoughts. Although this is scary to think about, there are things you can do to check in with your child.

The first step can be asking if they are having these thoughts. Research shows that asking them if they are having thoughts of hurting themselves does not “put the idea in their head.” It shows that you care about them and want to know that they are in a good mental headspace. It is important to let them know that you are always there for them even when things get hard for them. Teenagers will often act like they don’t need this from their parents but showing you are on their team can boost their morale. I think it is important to be a friend to your child and let them know that you are supportive of them and love them no matter what. Strong connection to a family member has actually been identified as a “protective factor” against suicide. Staying connected, staying positive, and being there to listen is crucial in keeping your child safe.

Lastly, if your child is having thoughts of suicide, reach out to a mental health professional or take them to the nearest emergency room.

Please reach out to our behavioral health team if you have questions or concerns about your child/ teen. We are available to talk with you about many mental health needs including suicidal thoughts, self harm, or severe depression. Our dedicated and experienced team are ready to help support you and your child through these scary thoughts.

5 Tips for a Great Start to the School Year

          1. Set a consistent bedtime (and wake up time). School age kiddos may need 9-11 hours and teens 8-10 hours of sleep a night. Make sure they’re getting exposure to sun

light in the mornings/early in the day to help reset that circadian rhythm, especially if they’ve been staying up late on summer nights.
2. Make sure kids have a balanced breakfast. Think a little protein and some complex carbohydrates (i.e. fruit). Try to avoid high sugar foods, as they may result in low energy levels later in the morning.
3. Touch base with your child’s teacher. Call, email, or meet in person early in the school year. Communication is key to advocating for your child, and teachers will appreciate your support.
4. Create a space for homework. Try to have 1 or 2 designated spaces that are quiet and stocked with needed supplies. Make sure your child is prepared when sitting down with a snack/drink and any work or supplies they may need for the day. Some physical activity, rest, or mindfulness activity may help them transition easier to homework time.
5. Encourage involvement in extracurricular activities. Participation in extracurriculars results in higher levels of academic achievement, contributes to character development, and results in better socialization, time management skills, and resiliency.

Mental Health Awareness Month

May is Mental Health Awareness Month! According to the CDC (Center for Disease Control), it is estimated that as many as 1 out of 5 children experience a mental disorder in a given year.

Has your child/teen been more stressed this month?

With MAP testing, KPREP, final exams, graduations – kids tend to put more pressure on themselves. Anxiety may look like fear or worry, but can also make children irritable and angry.

Anxiety symptoms can also include trouble sleeping, as well as physical symptoms like fatigue, headaches, or stomachaches. It’s normal to feel an increase level in anxiety in the last couple of weeks of school. With the school year coming to an end, help your child/teen take mental breaks from the stress.

Getting enough sleep: the importance of sleep goes beyond just the night before an exam – sleep empowers effective thinking and school performance.

Make sure your kids are eating a healthy diet: have them take a break from studying and help you prepare dinner.

Physical activity: the weather is getting warmer – take the dog for a walk, dance to your favorite song or go on a hike and enjoy nature.

Fostering Resiliency in Children

There has been a lot of talk about resilience and how important it is for the health and wellness of our children, families, and communities.

It is often said that ‘children are resilient’, and this is true, but what is not typically discussed are the factors that contribute to resilience. Simply put, resiliency is the ability to bounce back, manage adversity, and heal and learn from stressors.

Phillip A. Fisher from the University of Oregon Social Learning Center describes resilience as “The counter balancing of difficult things that may exist in a child’s life with the positive things that occur within the family and even positive things that may exist in the community.”

Positive interactions at home, school, and in the community help children to build confidence, feel in control, reduce stress, improve self-regulation, and develop prosocial character traits.

Positive interactions include:

           Snuggling

           Reading

           Going on a walk

           Watching TV

Noticing a difference in emotion (whether positive or negative)

Talking about day to day experiences

           Cooking/baking together

Doing arts and crafts, and more

As an adult, try to be aware of how you are responding to day to day and more significant stressors. When adults respond to stressors calmly, they are modeling composure, and in the future when there is a stressful event, the child will have tools to manage stress. If your family has recently experienced toxic stressors, and you are concerned with how your child is coping, do not hesitate to reach out to your primary care provider, or make an appointment with the behavioral health team!

Dealing with Anger

Everyone gets angry, but figuring out what to do and how to manage those angry feelings can sometimes be a little bit difficult, especially for younger kids.

Understanding our anger warning signs is a great first step in better managing the symptoms of anger such as yelling, throwing things, hitting, etc.

Warning signs are all of the things that we see and feel within our bodies that alert us that we are starting to get a little upset. Anger sometimes works on a scale and it can be beneficial to recognize our low, medium, and high warning signs.

Some examples might be:

Low- clench teeth, ball up fists, face gets hot

Medium- stomp feet, sighing or huffing, shaking

High- yelling, throwing items, hitting or kicking

A good activity to help younger kids recognize their anger warning signs is “Anger in the Mirror.” Next time your child is getting a little bit worked up, have them go to the mirror and look at themselves and point out all of the ways they can tell they are angry from their reflection.

Further, ask them questions about how they feel inside. Does their belly hurt? Do their ears feel hot? Is their head feeling funny? All of this can help you as the parent better recognize and intervene when your child is at a low anger level, and possibly prevent the anger from getting too high.

Another important reminder: Try your best to stay calm when your child gets upset, and understand that your child is experiencing a big emotion that they need your help in navigating.

Many times, we think of mental health therapy as trying to get comfortable on an office couch while we talk about our current day-to-day stressors or rehash past traumatic experiences in an effort to move forward, improve our functioning, and live our best lives.

Well, drop the “talk” out of it for our youngest and we can help our kiddos resolve their own stressors and negative experiences as well! Welcome to the world of Early Childhood Mental Health (ECMH), chock full of strategies from various modalities of therapy to help assist your littles in improving their functioning at home, daycare, school, etc.

Several of our counselors are skilled in ECMH and pull from a variety of modalities to work with our early childhood population and their families. Such modalities include Play Therapy, EMDR (Eye Movement Desensitization and Reprocessing), PCIT (Parent Child Interaction Therapy) CBT (Cognitive-Behavioral Therapy), and more!

So how does it work and what does it look like for our youngest children?

          PLAY THERAPY:

Virginia Axline, a psychologist and pioneer of Play Therapy, states: “play is the child’s natural medium of self-expression ... It is an opportunity which is given to the child to ‘play out’ his feelings and problems just as in certain types of adult therapy an individual ‘talks out’ his difficulties.” Gary Landreth, the grandfather of Play Therapy, says it best: “Play … expands self-expression, self-knowledge, self-actualization and self-efficacy. Play relieves feelings of stress and boredom, connects us to people in a positive way, stimulates creative thinking and exploration, regulates our emotions, and boosts our ego.”

So is it right for you? Does your child demonstrate any of the following issues at home, school/daycare, or in community settings? If so, mental health therapy can benefit you and your child!

One of Dr. Warrick’s many hobbies is being a small airplane pilot. Since 2017, Dr. Warrick has volunteered with the Civil Air Patrol, Northern Kentucky Senior Squadron. The Civil Air Patrol is the U.S. Air Force Auxiliary, a volunteer organization that serves communities in area of Emergency Services, Cadet programs (for youth ages 12 and older), and Aerospace Education.

Dr. Warrick originally joined the organization to be able to use his pilot skills to help with search and rescue, airborne photography, and other disaster relief missions. Dr. Warrick was selected to be the commander of the unit in August 2020.

At a recent awards event for all of Kentucky (known as the Kentucky Wing), Dr. Warrick was awarded Commander of the Year for his leadership of the Northern Kentucky Senior Squadron, and his squadron was recognized as the 2021 Kentucky Senior Squadron of the year as well. Congratulations to Dr. Warrick for his awards and thank you for your volunteer service to our community.

If you ever want to learn more about this organization, feel free to ask Dr. Warrick.

Parenting a Child With ADHD

The recommended treatment for ADHD is commonly a combination of medication and counseling for skill building. But parents can help out at home. Here are some suggestions to help create an environment at home to foster your child’s success with ADHD:

Keep it positive: ADHD can be frustrating for the child, parents, and siblings. When you are calm and focused, it is much easier to help your child be calm and focused.

Routine, routine, routine: Children and adults with ADHD benefit from knowing the time and place for everything. Using clocks and timers can help them manage their time. Visual aids like a chore list or checklist are helpful in to establish routines and as a reminder for daily activities.

Set the ground rules: Make sure to set clear expectations and rules.

Positive praise is a great tool to help reinforce positive behaviors. Immediate rewards and praise can work better than a promise of a future reward.

Get active – Keeping a child with ADHD busy is important. Whether it is an after school activity, a sport or a recreational activity with the family. Keeping active fosters improved social skills, improved self-control and helps grow self-confidence.

Most of all, be patient with your child and yourself and keep things in perspective.

Take care of yourself: Modeling behaviors such as healthy eating, exercise, and healthy sleep patterns can help your child learn great life skills and help you stay calm. Taking breaks when you get frustrated and seeking support can help the parent not feel so isolated and frustrated.
KHSAA Updated Guidance for Return to Sports After COVID

The Kentucky High School Athletic Association along with the Kentucky Medical Association has updated their guidance on returning to sports after COVID 19 infection, with the potential reduction of isolation from 10 days to 5.

The easiest way to count this is:

Day 0 is the first day symptoms occurred or a positive test if asymptomatic.

All symptoms (cough, shortness of breath, fever (≥100.4F), etc.) have resolved for at least 24 hours for mild cases or 7 days for moderate cases without the use of fever-reducing medication.

We can see you for the clearance exam on Day 5 or 6 as above. For example, let's say you have symptoms on Monday. We can see you in the office on Saturday and you can return to practice on Sunday.

Our Next Chapters

Greetings Pediatric Associates family. Pediatric Associates just celebrated its 30th birthday as a practice. We are celebrating this milestone with great optimism and energy, and we are honored to be a trusted part of your family.

Like all families, our Pediatric Associates family experiences changes and transitions. At the end of this year, on December 31, 2022, the three founding members of our practice, Dr. Cunha, Dr. Brokaw and I, Dr. Bolling, will be retiring from Pediatric Associates. Collectively, we have over a hundred years practicing pediatrics, and nearly all of it was here, at our beloved practice.

The three of us are very excited about being able to pursue interests that have been on the backburner for many years and to spend more time with our own families. Although we arrived at our decisions independently, we also feel a certain symmetry and poetry in retiring together.

Dr. Cunha, a New York native and educated in Florida, will be spending more time at his home in Florida, hoping for an opportunity to care for children less fortunate in Highland County, Florida. He is looking forward to traveling more with his family and friends. He will certainly be spending more time attempting to master golf, pickleball and fishing! Most importantly, he will be there for his wife, Donna, their four adult children and their young families. He is very anxious to spend more time with his granddaughter, Elliotte.

Dr. Bolling, an avid hiker and skier, plans to take advantage of having more time to explore and travel with his spouse, Stephen, and their dog, Murphy. He continues to volunteer with the American Academy of Pediatrics studying and teaching in the area of pediatric obesity. He is involved at his church and is looking forward to several projects that promote getting kids into nature. He is also excited about spending more time with his siblings and their growing extended families.

Dr. Brokaw looks forward to spending as much time as possible with his wife, Jill, and their three adult children. As all three live in distant parts of the country, Barry and Jill will enjoy traveling and exploring new areas, especially by hiking, kayaking, fishing and skiing. Also, he hopes to continue to rehab a family log cabin in northern Ontario. Consequent to his love of nature, he hopes to promote outdoor experiences to youth who might otherwise never have the opportunities. “Dr. Barry” hopes to stay active in his church and volunteer with organizations such as Redwood.

Our partners here at Pediatric Associates are way more than work colleagues. They are our friends, confidantes and counselors. They will continue to meet all your healthcare needs. The practice has always prided itself on having the most talented pediatric providers in the region, and that commitment to excellence is stronger than ever.

This new chapter for you, our families, is also exciting. Nothing has given the three of us greater joy than growing up with your families. With change comes opportunity and growth. We are very grateful for the trust you have placed in us over the decades, and we are grateful to our colleagues at Pediatric Associates who will be there with you and your families moving forward.

Dr. Cunha, Dr. Brokaw and Dr. Bolling

Winter Family Activities

Hi all, Megan here – the Early Childhood Counselor here at PANK. I know we’re all struggling with this recent drop in temperatures and feeling contained within our homes to avoid those blustery winds and cold chills.

If you’re looking for ways to nurture positive relationships with your kids as we’re all stuck inside together during these cold wintry days, look no further! There are plenty of ways to engage with your kids in positive, fun, and even helpful ways, all in the comfort and warmth of your own home! Here are a few of my favorite suggestions!

          FAMILY GAME NIGHT:

Pull up a chair, or two or three or four, and get the family involved in good old-fashioned fun with board games or card games! This is an excellent way to connect with our kids, outside of screens, and even relive some of our favorite moments from our own childhoods. (Trigger warning for those who are still sore about past Monopoly games...) Use of games can help practice frustration tolerance, problem-solving, and appropriate social skills, such as taking turns and patience! You can even throw a fun mix into classic games, such as Uno and Candy Land, to explore feelings or life experiences with your kids.

          COOK/BAKE TOGETHER:

Who doesn’t love a helping hand around the house? Cooking or baking with kids is a great way to teach life skills, as well as facilitate practice of following directions, self-control, patience, and time management in a fun and creative way! Not to mention, it ends with a yummy meal or treats. Pull up some of your old family recipes to bond over, or have fun seeking a new idea to try and seeing how it turns out!

          FAMILY MOVIE NIGHT:

Snuggling up on the couch to watch a family favorite or checking out the newest release on Netflix can be a great way to connect and relax with your family. Remember to be present in the moment – all other electronics would preferably be put aside to enjoy that moment together! Make some snacks or grab a bag of popcorn from your local theater to pizzazz it up even a little more! This shared experience can allow for further connection and conversations about the movie as you share about your favorite characters, moments, songs, etc.

          ARTS AND CRAFTS:

Why not brush up on your artistic side with some art activities? These can be as old-school as sock puppets for a puppet show, as intentional as some homemade balloon stress balls or sensory glitter bottle for a calm-down tools, or as heartwarming as some self-depicted family portraits that will definitely be fridge- (or frame!) worthy! Letting kids explore their artistic side is liberating and fun for self-expression. And it’s okay for us, as adults, to get messy too once in a while!

None of these up your alley?

Try instead some puzzles, hide and seek, scavenger hunts, pizza parties, or just draft a to-do list for warmer days ahead when we can get back outside and feel the warm sunshine! As always, feel free to reach out for any and all of your parenting needs, and

          STAY WARM

          !

Update on COVID 19 Testing and Quarantines

Currently we are seeing high numbers of COVID cases and there has also been a recent increase in Flu in our region. If your child has symptoms, and if you have done an at home rapid test which is positive, then this is consistent with being positive for COVID and there is no need for further testing. If you have symptoms but have not yet been tested we recommend scheduling a visit in the office. It is important that you follow current isolation and quarantine guidelines for your child and family.

The CDC recently updated their guidance for covid isolation and quarantine, with a shortened duration to 5 days followed by a period of continued masking and monitoring of symptoms. However, the local health department, and many school districts have not yet changed their policies to reflect these new recommendations. Therefore, we recommend that if someone in your house is positive, and you have questions about when your child can return to school, it is best to check with the appropriate person at your child's school such as the school nurse, daycare center director, etc.

The minimum timing between first and second dose is 3 weeks and between primary series and booster is 5 months (2 months if first dose was Johnson & Johnson product). Please call the office to schedule. We encourage everyone that is eligible to get vaccinated to protect yourselves and your loved ones.

Many families have insurance changes effective Jan 1 each year. If your insurance has changed, please notify us of this change when you schedule your appointments and make sure we are still in your network. You may also email your new card to our billing department at billing@pediatricassociatesnky.com. Please bring your card to all appointments. Also, most high deductible health plans reset deductibles as of Jan 1. Our policy for patients covered by high deductible plans is to collect partial payment for ill visits at the time of service.

We strongly endorse you giving your child this thoroughly-tested, safe and effective vaccine. Never hesitate to ask any of us in the office about this important medical advance that helps reduce illness in both kids and adults. We are giving the vaccine at all three offices, but by appointment only due to limited supplies. We continue to give the vaccine to our older patients as well. Call us today to schedule!
