We Accept Most Insurance Carriers

Please have your card with you at EACH VISIT. If we file your claim, we ask you to pay your copay at the time of the visit.

Due to insurance requirements, co-payments are due at check-in with your appointment.

If you have a high deductible insurance plan, we ask you to pay at least $75 toward your deductible at the time of the visit instead of the full amount of the visit until your deductible is met. We will then bill you for the balance after insurance responds.

Please make sure that you have your infant’s insurance card by the 2 month visit. If this is not possible, please call the office BEFORE the visit.

It is your responsibility to make sure we are participating with your Insurance plan. Due to the many different plans within Insurance Companies please call your Insurance Company to make sure we are participating in your individual plan.

If we are not a participating provider for your insurance, if you are not insured, or we do not file with your insurance company, you may be responsible for the entire charge at the time of service.

Credit Card on File

Wouldn’t it be nice if paying bills was a little easier? With credit card on file it is. We partner with Instamed to help make paying your bill faster and easier. 

Credit card on file is a secure way for you to authorize us to charge you if your outstanding balance for a visit that is less than $100. We let you know we put a charge through so there are no surprises on your credit card statement. To utilize credit card on file you need to sign the below form and contact us with your credit card information. No credit card information is kept at our office, it is all secure through Instamed. E-Statements

To sign up for e-statements you can log into Instamed. From here you can sign up or log in. Once logged in you will see a page that looks like this. If you click the enroll in paperless statements button in the top right corner you can switch from paper to e-statements. In the future, you will be able to easily log in and pay your bill from the email when it arrives.
